Объяснение эффективности



 

С точки зрения теории науки описать терапевтические методы — значит изобразить их в плоскости технологии. Технологии — это мероприятия, предлагаемые для возможно более продуктивного достижения цели. Поэтому решающим критерием оценки является даже не истинность (в отличие от теорий), а скорее эффективность и рентабельность (ср. главу 4).

Если мы полагаем, что методы поведенческой терапии доступны эмпирической проверке, то автоматически встает вопрос: как и почему они действуют или соответственно какие элементы комплексной терапевтической программы ответственны за происходящие изменения, т. е. встает вопрос о факторах терапевтического воздействия.

Как уже говорилось в главе 22.1, теоретическое обоснование терапевтических процессов должно предлагать объяснение человеческого поведения и его изменения во времени, т. е. в любом случае учитывать компоненты научения.Поведенческая терапия имеет в этом отношении то преимущество, что она изначально развивалась на основе теорий научения и к тому же имеет многочисленные содержательные связи с другими частными психологическими дисциплинами, равно как и со смежными дисциплинами (например, исследование когниций в общей психологии, теории атрибуции в социальной психологии, психологии развития, психофизиологии, теории эволюции, биохимии, неврологии и т. д.; см. здесь O'Donohue & Krasner, 1995). Эта особая близость поведенческой терапии к научной психологии выражается хотя бы в том, что уже названия некоторых методов указывают на эмпирически оправданные, обусловившие их появление теории (например, научение по моделям), даже если мы более дифференцированно рассматриваем сегодня отношения между фундаментальными теориями и технологиями (ср. главу 4).

В технологической плоскости в исследовании терапии различают неспецифические, или свойственные всем методам, факторы терапевтического воздействия (например, стратегии ведения беседы, ролевое структурирование и т. д.) и специфические (например, отдельные методы или техники) (ср. главу 22.1). В практике же на вопрос о том, что именно отличает один фактор как «специфический», а другой как «неспецифический», можно ответить как угодно, но только не однозначно. Чтобы приблизиться к ответу, необходимо в каждом случае наряду с описанием самого фактора учитывать и то, что мы знаем о его воздействии, и его теоретическую подоплеку. Эти эмпирические и теоретические выкладки по поводу отдельных факторов воздействия делают крайне относительной (во всяком случае, для поведенческой терапии) разницу между специфическим и неспецифическим механизмами. Если мы, объясняя отдельные этапы терапевтического образа действий, опираемся на психологические представления о моделях, то это еще не говорит ни о совершенстве, ни об исключительности этого объяснения, а тем более окончательного обоснования.
